At Iconic Medical Arts, our mission is to provide elevated, physician-led aesthetic and wellness care grounded in safety, expertise, and personalized attention. Our practice policies ensure a consistently smooth, respectful, high-quality experience for each patient.
Please review the following guidelines prior to your appointment.
A minimum of 24 hours’ notice is required for all appointment cancellations or rescheduling.
Cancellations made within 24 hours of the scheduled time – or failure to attend your appointment – will result in a $50 missed appointment fee for standard appointments and a $100 missed appointment fee for procedure appointments
Patients arriving more than 10 minutes late may need to be rescheduled and a shortened appointment or missed appointment fee may apply.
Iconic Medical Arts operates as a fee-for-service medical practice and does not accept insurance.
All new patients meet with the physician for an initial consultation, which includes:
Consultation fees vary depending on the type of medical evaluation (aesthetic, musculoskeletal, hair restoration, cannabis consultation, sexual etc.).
All treatments are performed or clinically supervised by our physician.
Dosing, syringe amounts, and treatment recommendations are based on anatomy and medical judgment rather than predetermined quantities.
Payment is required at the time of service.
We accept major credit cards, HSA/FSA cards (where applicable), and cash.
Pricing and promotional offers are subject to change without notice.
Due to the medical nature of our services:
Your safety is always the first priority.
The physician may decline or postpone treatment when medically necessary.
Additional testing, evaluation, or clearance may be required prior to certain procedures.
Patients are responsible for following all pre-treatment and post-treatment instructions provided by the physician or clinical staff.
Failure to follow instructions may impact both safety and treatment outcomes.
Touch-ups for neurotoxin treatments (such as Botox, Xeomin, Dysport and Daxxify) are not included and will be billed separately if additional units are required
To maintain a safe and professional environment:
Clinical photographs are required for accurate medical documentation.
Additional consent will be obtained for any images used for educational or marketing purposes.

By receiving care at Iconic Medical Arts, you consent to being contacted by phone, text, email, or patient portal for appointment reminders, treatment instructions, medical updates, or follow-ups.
We strictly adhere to HIPAA regulations.
Your personal health information will not be released without your written consent unless required by law.
